Status: Full-time position
Location: Denver, CO
Job Summary: Provide direct mental health clinical assessments and treatment to the severe and persistent mentally ill, including evaluations, monitoring treatment plans, and individual therapies. Working closely with treating Psychiatrists. Targeted population for this position is adults Enhanced Outpatient. Responsible for managing care load up to 35 adult consumers residing in nursing homes within the Denver area.
Qualifications: Master's Degree in social work.
Current license as a Clinical Social Worker in the State of Colorado.
Duties: Provide psychological evaluations/problem identification, crisis interventions, and individual, group and/or family therapy to assigned consumers.
Provide case management services to consumers as needed.
Develop treatment plans; monitor treatment and follow-up at disposition times.
Assist consumers with psychological development, social development, and rehabilitation.
Coordinate consumer's use of other MHCD programs and outside community resources.
Coordinate medication/medicinal needs of consumers with medial sources.
Manintain accurate and timely clinical records consistent with MHCD standards.
Coordinate consumer billing and modify treatment program in accordance with insurance coverage, plan provisions, and individual ability to pay.
Participate in team/MHCD meetings, in-services, and supervisory sessions as required.
Perform other duties as assigned.
